"Spelling, capitalization"
Marc, if you, or anyone else is curious, I chose sentence-style capitalization for the title instead of headline-style because sentence-style is, increasingly, the norm in most places outside the US, and I was trying to make our wiki less US-centric.
But I don't care (much) which way we do it. Just wanted to add an explanation, for future reference.

Procedure to add practitioners?
The page invites practitioners to add themselves. Is it acceptable to add others?
A notable omission from the list is Dr. Lokken in Alabama:
